What this site collects
Interest forms collect your name, email address, selected interest, and an optional organization name for partnership inquiries. Contact forms also collect the message you choose to send. We store the submitted information, a consent record, submission time, and follow-up status. Please do not send diagnoses, medical histories, or other health details.
Why we collect it
We use contact information to manage interest in the developing initiative and to follow up about the subject you select. A form submission does not enroll you in medical care or confirm a place at an event. We do not automatically send email from these forms.
Storage and access
Submissions are stored in the website’s hosted database and are available to the designated founder in a protected inbox. Hosting providers process information needed to operate the site. No submission directory is public. This site is not designed to store protected health information or clinical records.
Sharing and your choices
We do not sell contact information or share participant lists with sponsors for their marketing. You may ask to stop contact, access or correct your submitted information, or request deletion using the contact form and selecting “Privacy request” or “Contact preferences.” You can also reply to a follow-up email from our team. Requests are reviewed by the founder and may require identity verification before action.
Retention
Contact records remain until removed by the founder. This initiative does not yet apply an automatic deletion schedule. The founder can permanently delete a record from the inbox; prior emails and any downloaded contact exports must be handled separately.
Device storage
The appointment checklist stores only checkmark selections in your browser. You can clear them with the reset button or by clearing your browser’s site data. We do not use advertising trackers or analytics scripts in this version.

Resource links lead to independent organizations with their own privacy policies. We are not affiliated with those organizations simply because we link to them.
Peer support and medical boundaries
Our planned peer community is not therapy or medical care. Participants will be asked to respect privacy, but confidentiality in a peer group cannot be guaranteed. Public storytelling will require separate permission. Do not use this site or its forms for urgent medical needs. Call 911 in an emergency.
